We spend a lot of time managing risks. We build risk registers, assess probability and impact, and define mitigation plans. And yes, this is essential. But most project failures I’ve seen didn’t happen because a risk materialized. They happened because people chose to believe in a convenient story. An optimistic schedule everyone knew was unrealistic. A budget approved just to make the business case more attractive. A scope that only works on paper. That’s not risk management.
